UNDERGRADUATE PROGRAM (Bachelor’s Degree, graduate of SHS)

For Senior High School Graduate

1. Applicant Photo
2. Certificate of Good Moral Character
3. Senior High School Card
4. PSA copy of Birth Certificate
5. Copy of vaccine card

 

For ALS Passer (SHS Level Certificate)

1. Applicant Photo
2. Certificate of Good Moral Character
3. Certified True Copy of List of Passer
4. Certificate of Rating with remarks equivalent to senior high school level/graduate and date of graduation
5. PSA copy of Birth Certificate
6. Copy of vaccine card

 

For Transferee AVAILABILITY OF SLOTS

1. Applicant Photo
2. Certificate of Good Moral Character
3. Copy of Grades / Transcript of Record (Must have NO dropped and failing grade)
4. Honorable Dismissal
5. PSA copy of Birth Certificate
6. Copy of vaccine card

 

GRADUATE PROGRAM (For Masters and Doctoral, graduate of bachelor’s degree)

1. Honorable Dismissal
2. Transcript of Record
3. Permit to Study
4. PSA copy of Birth Certificate

For Professional Education (For Graduate of Bachelor’s degree)
